Stratum’s Connexis Physicians@Home to provide care at Aviva campus
SARASOTA – Stratum Health System and Aviva – A Campus for Senior Life are proud to announce an agreement that designates Stratum affiliate Connexis Physicians@Home as the preferred provider of medical services throughout the Aviva community.
The contract between Stratum and Aviva went into effect Oct. 1, 2019. Connexis Physicians@Home now provides medical services at each of Aviva’s communities: Kobernick Independent Living, Anchin Assisted Living and Memory Care and Benderson Rehabilitation and Skilled Nursing.
A Connexis physician serves as the medical director for Benderson Skilled Nursing, and a Connexis Nurse Practitioner directs a medical clinic on the Aviva campus, assists the Connexis physician and provides resident health education.
The Stratum Health System family of companies is a nonprofit, integrated network providing care and support to people facing chronic or advanced illness. Other companies within the Stratum family are Tidewell Hospice, Avidity Home Health and Approved Home Health.

Connexis Medical Services features three community healthcare offerings, Connexis Physicians@Home, Connexis Palliative Care and Connexis Physician Services. Connexis Physicians@Home brings expert, primary medical care directly to patients through visits to private homes and assisted-living and skilled-nursing facilities.
